NHS registrations can only be made by completing the form below. Please do not call the practice if you are new to us and wish to register as an NHS patient.
Once we receive your form, we will call you back to arrange your New Patient Examination.
Please note: your NHS registration will only be complete once you have visited our practice for your New Patient Examination.
You will be asked to complete a medical history form when you arrive, so please bring a full list of any medication you are taking.
You will also be asked to sign an NHS Practice Record (PR) Form. If you are exempt from paying NHS dental charges for any reason, please bring evidence of this with you. Further details are available from the NHS BSA website.
